Policies and Procedures

THE FOLLOWING IS A DETAILED LIST OF OUR POLICIES.

BY SIGNING THE REGISTRATION FORM, ALL PARTIES AGREE TO THE POLICIES LISTED BELOW:


All tuition is due the first lesson of every month. I understand that payment may be brought or mailed to the studio. I understand there is a discount when paying with cash or a check. Should I choose to pay through an app or with a credit card, I will not take the discounted price but pay the full tuition price. I will be responsible for all balances should the incorrect amount be paid.


Tuition is based on a 9-month schedule, divided conveniently into 9 equal payments. I will pay the same monthly amount regardless of the number of classes that month beginning in September and concluding in May. Should there be a need to continue classes into June a partial payment may be added. You will be notified in advance of such charges if necessary.


All tuition payments, registration fees, costume and dancewear costs and competition fees are non-refundable regardless of any situation.

I understand that any tuition not paid by the 10th of every month will incur a late fee of $25.00. THERE ARE NO EXCEPTIONS. If a different payment plan needs to be made, I will make the proper arrangements at the first lesson of the year. CFDA II is happy to accommodate those who need a different schedule or other arrangements.


I understand that there is a returned check fee of $35.00 for any returned checks. After 2 returned checks, I no longer have the privilege of writing checks and must pay cash from that point forward.


Each class a dancer participates in will need a costume for our recital. There is a one-time, non-refundable fee due by the end of October. If your payment is not received, no costume will be ordered therefore leaving you open to extra fees if ordered separately. Please ask your instructor how many costumes you will need in a combination class, as this may differ.


CFDA II will send costumes home with each dancer from the studio in early spring. This date will be determined at a later time, and you will be notified in advance. If there is a balance due on my account (including past due tuition, late fees, dancewear, etc.), I understand that I am not entitled to pick up the costume until I have paid the balance due in full.


I understand that my child’s instructor will be happy to speak with me about my child whenever necessary. However, it is appreciated if it is done at a time that does not interfere with any classes. Phone numbers and email addresses have been provided. All tuition and cost questions should be directed towards Miss Lisa only.


I understand that I am entitled to make up classes when a class is missed or cancelled due to inclement weather, illness or any other reason. A make-up schedule will be provided. Any cancellations due to the weather do not result in credits to my account. I have the option to attend a make-up class from the schedule I have been given.


CFDA II has the right to publish any student pictures and names related to the studio in print work or the internet unless written request is received.

CFDA II or any of their instructors are not responsible for injuries/illnesses that may occur at the studio or any CFDA II related function. I assume all risks related to dance.


I understand and agree to follow the CFDA II dress code. Failure to do so will result in my child not participating in class. All students are required to have the proper dance and footwear by the 3rd lesson of the year.


I understand my student will not leave the studio without proper cover up clothing.


I understand that my child is not allowed to leave the studio unattended unless CFDA II instructors are notified by a parent/guardian. All dancers should be met at the door or in the waiting room for pickup by parent/adult responsible.


CFDA II will not be responsible for lost, stolen or broken personal items.



My family will respect the property of CFDA II, be it during class, in the waiting area or on any grounds associated with CFDA II or a reated performance venue.


I understand that it is important for my child to participate in all scheduled classes. I will notify the studio in case of any absences.


I have read the above studio policies and understand them completely. I have chosen to comply with all the policies in place at THE CENTER FOR DANCE ARTS II. By signing the registration form, I agree to all terms mentioned above. I will keep this page for my records.
